HERE ARE THE RULES:
Collect 2 nickels every appointment if ALL FOUR of the following
Categories are met
Collect 1 nickel every appointment if 2-3 categories are met.
* On Time for Appointments
* Good Oral Hygiene
* Full Time Elastic Wear, Head Gear Wear, or other Appliance Wear
* No Broken Bands, Brackets, Broken Appliances
No Nickels Earned if:
*You Missed Your Last Appointment (without 24 hours notice)
*Any Emergency visit
*Broken or Lost Appliances
Bonus Nickels
This is a great way to earn extra nickels
1 Nickel for making your next appointment before leaving the office
2 Nickels when you bring a signed form from your dentist saying you had good hygiene at your last cleaning (ask us for a form) and 3 additional nickels if you had excellent hygiene AND no cavities
2 nickels for all B’s or better on your report card (bring your card to show us)

10 Nickels = $5 McDonalds or/Starbucks or/ Jamba Juice card
15 Nickels = Blockbuster or/ Hollywood Video card
20 Nickels = $10 Toys-R-Us or/ Cold Stone Creamery or/ iTunes card
30 Nickels = 2 Movie Tickets
40 Nickels = $20 iTunes Card
50 Nickels = $25 gift card for Game Stop or/ Disney Store or/ Old Navy
60 Nickels = iPod Shuffle/ $50 Target Gift Card or Sonicare Toothbrush System
75 Nickels = Nintendo DS or / 2 Great America Passes
100 Nickels = Xbox 360 or Nintendo Wii
- patients who are currently in active treatment (not retention) may participate
- if we forget to give you your nickels please remind us---we won’t be able to give them at the next appt.
- nickels are not combinable between family members
